Plan Overview
This is an annual reduced-fee dental plan exclusively for our existing patients, offering access to high-quality dental care at discounted rates from our usual, customary, and reasonable fees. It is important to note that this is not an insurance plan, so no claims will be processed, and no payments will be made to any other healthcare providers. This plan cannot be combined with other discounts, insurance plans, CareCredit, or promotional offers.
Membership Terms
- Enrollment fees must be paid in full before any dental care under the plan can be received.
- The plan is valid for one year from the date of enrollment fee payment.
- Unused benefits during the active period will not carry over to the next year.
- The membership is non-refundable and does not offer additional discounts.
- Members are responsible for paying for treatment and services in full on the date of service to receive the plan’s discounts.
- Fees for dental services may change throughout the year.
- Members are responsible for scheduling and attending all appointments included in the plan. To avoid missed appointment fees, please provide at least 48 hours’ notice for any appointment changes.
